İntraabdominal basınç (AB) artısları sonrasında sekillenen intraabdominal hipertansiyonun (AH) degisik organ ve sistemlerde olumsuz etkilerinin oldugu ve bu olumsuz etkilerin düzeltilmedigi durumlarda mortalitenin yüksek oldugu bilinmektedir. Bu çalısmada köpeklerde CO2 insüflasyonu ile olusturulan 25 mmHg AB'nin intestinal doku iyilesmesine ve bakteriyel translokasyon (BT)'a etkileri ile 30. dakikada uygulanan abdominal dekompresyonun etkileri arastırıldı. Gruplar, kontrol grubu (G-K, n=6), AH grubu (D-I, n=6) ve AH ve abdominal dekompresyon grubu (D-II, n=6) olarak belirlendi. Tüm gruplarda ileumda olusturulan enterotominin onarımı tek kat basit ayrı dikislerle yapıldı. AB ölçümleri intravezikal ölçüm yöntemi ile gerçeklestirildi. Hemogram degerleri çalısmadaki tüm deneklerde operasyon öncesinden baslayarak deney sonuna kadar, arteriyel kan gazı degerleri ise karın boslugunun kapatılmasından sonraki süreçte degerlendirildi. BT aranması operasyon sonrası 24. saatte yapılan re-laparotomi sırasında alınan mezenteriyel lenf nodüllerinden yapılan aerob ve anaerob mikrobiyolojik inceleme ile gerçeklestirildi. ntestinal doku iyilesmesi histopatolojik olarak degerlendirildi. Histopatolojik degerlendirme kapsamında histomorfometrik olarak da bagdokunun hücresellik derecesi, reparasyon alanındaki yangı siddeti ve kapillar damar rejenerasyonu kriterleri kullanıldı. Çalısmada AB artısının arteriyel kan gazında belirgin sekilde pH ve laktat seviyelerinde azalmaya ve PaCO2'de artmaya neden oldugu belirlendi. Tüm gruplarda BT gözlenirken, bakteri sayısının AB artısı olusturulan gruplarda kontrol grubuna göre daha fazla oldugu ve BT'ye ugrayan bakterilerin çogunun Gram(-) enterobakteriler oldugu belirlendi. ntestinal doku iyilesmesinin kontrol grubuna göre D-I grubunda çok belirgin bir sekilde geciktigi, D-II grubunda ise iyilesmedeki gecikmenin daha hafif oldugu belirlendi. Sonuç olarak bu çalısma, köpeklerde olusturulan 25 mmHg'lık AB artıslarının asidoz ve hiperkarbiye neden oldugu, BT'yi siddetlendirdigi ve intestinal doku iyilesmesini geciktirdigi, erken abdominal dekompresyon uygulanmasının ise bu olumsuz etkileri hafiflettigini göstermistir.
Due to the increase in intraabdominal pressure (IAP), intraabdominal hypertension (IAH) has an adverse effects on various intraabdominal organs and systems. f this complications are not treated, it is known that the mortality rate will reach very high values. This study is performed in dogs to evaluate the effects of IAP of 25 mm Hg (reached by CO2 insufflation) on intestinal wound healing and bacterial translocation and the effects of surgical decompression performed after 30 minutes. The first one was the control group (C-K, n=6), the second IAH group (D-I, n=6) and the third group was the IAH and abdominal decompression group (D-II, n=6). In all groups enterotomies made in ileum were sutured with one layer simple interrupted suture. IAP was performed by CO2 insuflator. IAP was measured via bladder pressure measurement method. Since the hemodynamic parameters in all dogs were evaluated before and during the research, the arterial blood gas values were assessed after the closure of abdominal cavity. In order to analyse bacterial translocation, twenty-four hours after operation dogs underwent relaparotomy through the previous incision and mesenteric lymph nodes were taken for aerobic and anaerobic microbiological analyse. Intestinal wound healing was assessed histopathologically. In this histopathological evaluation also connective tissue cellularity degree, inflammation degree in reparation place and capillar blood vessel regeneration criteria were assessed histomorphometrically. In this study by increase of IAP in D-I and D-II groups, while pH and lactate levels in arterial blood gas values were decreased significantly, the PaCO2 value was increased. Bacterial translocation was observed in all groups. The counts of bacterial population in groups with IAP was detected higher comparing to the control group and most of the bacteries which formed bacterial translocation were found to be the Gram (-) enterobacteries. Intestinal wound healing in D-I group compared with control group was significantly delayed, but the healing in D-II group was delayed less than the others. In conclusion, the present study showed that IAP of 25 mm Hg in dogs cause asidosis and hypercarbia, also deteriorates the bacterial translocation and delays the healing time and with early abdominal decompression the adverse effects of IAP can be decreased.
