İskemi-reperfüzyon hasarı ve buna baglı hayati organlarda olusan fonksiyon bozuklukları insan hayatını ciddi boyutta etkilemektedir. Birçok sebebe baglı iskemi olabilir. Özellikle insandaki aort cerrahisinde suprarenal aort klempajından sonra olusan iskemi ve sonrasındaki reperfüzyon bilhassa böbreklerde ciddi yetmezliklerle sonuçlanabilen reperfüzyon hasarları olusturabilmektedir. Birçok çalısmada gösterildigi gibi iloprost; antiagregan, sitoprotektif, antioksidan ve vasodilatör etkilere sahiptir. Bu çalısmada iloprost'un , böbrekte iskemi ve ardından reperfüzyon ile meydana gelen hasarlanmaya karsı koruyucu etkilerini serumda üre, kreatinin, sistatin c ve beta2 mikroglobulin'e bakarak arastırmayı amaçladık. Çalısmamızda ratlar kullanıldı. 8'erli 4 grup olusturuldu. Sham, RIR, Sham+iloprost ve RIR+iloprost. İskemi olusturulan gruplarda 60 dakika iskemi ve 120 dakika reperfüzyon uygulandı. Sham grubunda sadece batın açıldı, RIR ve RIR+iloprost gruplarında batın açıldı ve sol böbrek arterine klemp kondu. Bütün gruplardan kan alındı ve serumlarında Üre, Kreatinin, Sistatin C ve Beta2 mikroglobulin çalısıldı. Sonuçları gruplara göre karsılastırıldı. Sham, RIR, Sham+iloprost ve RIR+iloprost grupları mukayese edildiginde, iloprost'un böbrekler için zararlı etkisinin olmadıgını, uyguladıgımız iskeminin böbrekte hasar olusturdugunu tespit ettik. Kullandıgımız iloprostun ise böbrekte bu hasarlanmada hasarı azaltıcı etkilerini tespit ettik. Zira üre, kreatinin ve Sistatin C düzeylerini bu hasarı azaltmada anlamlı olarak farklı bulduk. Beta2 mikroglobulin de ise bu anlamlı farkı bulamadık. İskemi öncesinde 20ng/kg/dak'dan verdigimiz iloprost'un , böbrekte iskemi ve ardından reperfüzyon ile meydana gelen hasarlanmayı azalttıgını gruplar arası mukayesede özellikle üre, kreatinin ve Sistatin C degerleri dogrultusunda anlamlı olarak tespit ettik. Böbrekteki hasarlanma ve iloprost'un etkilerini beta2 mikroglobulin sonuçlarına göre degerlendiremedik 54 ve bu durumu beta2 mikroglobulinin özellikle ileri evre böbrek hasarlanmasında daha duyarlı olmasına bagladık.
Ischemia-reperfusion damage and its consequences due to functional malformations in vital organs affect human life seriously. Ischemia may be caused by a number of reasons. Especially ischemia and reperfusion after suprarenal aortic clamping in human aortic surgery may cause reperfusion damage which may even lead to renal insufficiency. As shown in some studies, iloprost has all antiaggregant, cytoprotective, antioxidant and vasodilator effects. In this study, we tried to investigate protective effects of iloprost in damaged kidneys after ischemia and reperfusion by using serum urea, creatinine, cystatin C and Beta 2 microglobuline. In this study we used 4 groups of rats, and 8 rats were present in each group. The groups were sham, RIR, Sham+iloprost and RIR+iloprost. In ischemia group, we performed 60 minutes of ischemia followed by 120 minutes of reperfusion. In the Sham group, only abdominal exploration was performed, whereas in RIR and RIR+iloprost groups the abdomen was opened and left renal artery was clamped. Blood samples were taken from all of the groups and serum urea, creatinine, cystatin C and Beta 2 microglobuline levels were evaluated. The results were compared in accordance with the groups. When the sham, RIR, Sham+iloprost and RIR+iloprost groups are compared, we?ve found that iloprost do not have harmful effects on kidneys and the ischemia we?ve created performed renal damage. We have seen that iloprost have protective effects in case of decreasing renal damage. We?ve found statistically meaningful values of urea, creatinine and 55 cystatin C in decreasing the renal damage. We could not find any statistically meaningful differences in case of Beta 2 microglobuline levels. We?ve found that iloprost infusion of 20ng/kg/min before ischemia, decreases renal damage caused by ischemia and reperfusion injury, especially when compared to the results of groups of serum urea, creatinine and cystatin C. We could not evaluate the renal damage and effects of iloprost in case of Beta 2 microglobuline results and decided that this might be due to Beta 2 microglobulin?s more specificity to especially to end stage renal disease.
