Bu çalısma, Selçuk Üniversitesi Meram Tıp Fakültesi Kadın Hastalıkları ve Dogum Anabilim dalında, riskli ve risksiz gebeligi olanlarda sosyodemografik özelliklerin, depresif semptom görülme düzeylerinin, yasam kalitelerinin ve bunların birbirleriyle olan iliskilerinin arastırılması amacıyla yapıldı. Bu çalısmanın evrenini, Selçuk Üniversitesi Meram Tıp Fakültesi Kadın Hastalıkları ve Dogum Bölümü'ne 15 Subat-31 Agustos 2006 tarihleri arasında basvuran 297 gebe olusturdu. Arastırmacı tarafından kadınlara bilgi verilip rızaları alındıktan sonra sosyodemografik bilgi formu, WHOQOL-BREF (TR) ve Beck Depresyon Ölçegi, yüz yüze görüsme teknigi ile uygulanıp verilen cevaplar ankete arastırmacı tarafından kaydedildi. Beck Depresyon Ölçegi (BDÖ) kesme puanı 17 olarak alındı. Çalısmamıza katılan gebelerin %34.4'ünde (n=102) ve riskli gebelerin %42.6'ında (n=49) BDÖ düzeyleri 17 ve üzeri olarak bulundu. 17 ve üzeri BDÖ düzeyi olanlarda riskli gebelik görülme durumu, 16 ve altı BDÖ düzeyine sahip olanlara göre istatistiksel açıdan anlamlı derecede yüksek bulundu (p=0.017). Riskli gebelik durumu esleri ilkokul, ortaokul ve lise mezunu olan gebelerde esi üniversite mezunu olanlara göre 3?4 kat [sırası ile OR, %95 CI; 3.060 (1.430-6.549), 4.474 (1.840-10.879), 3.222 (1.546-6.714)]; esleri isçi, serbest ve issiz olan gebelerde esleri memur olan gebelere göre yaklasık 1.7-2.8 kat [sırası ile OR, %95 CI; 2.804 (1.382-5.689), 1.853 (0.955-3.598), 1.679 (0.434-6.495)]; BDÖ düzeyi 17 ve üzeri olanlarda 16 ve altı olanlara göre 1.8 kat artarken [OR=1.807, %95 CI; (1.108-2.946)]; çalısan gebelerde çalısmayan gebelere göre yaklasık 3 kat azalmakta idi [OR %95 CI; 0.316 (0.134-0.744)]. Riskli gebelerin fiziksel saglık (p<0.001) psikolojik saglık (p=0.036), sosyal iliskiler (p<0.001) ve genel (p<0.001) alanlarda yasam kalitesi puanları, risk tasımayan gebelere göre istatistiksel açıdan anlamlı derecede düsük bulundu. Çalısmamızda riskli gebelerin %42.6 gibi yüksek bir oranında depresyon belirtilerinin ortaya çıktıgı ve riskli gebeligin yasam kalitesini düsürdügü görüldü. Gebelik depresyonun erken tanısı ve tedavisi anne ve bebek açısından çok önem tasımaktadır. Hekim tanı koyar koymaz kar-zarar oranını degerlendirerek tedavi verip vermeyecegine karar vermek durumundadır.
This study is constructed to search the socio-demographical characteristics, levels of depressive symptomatology and quality of life and the relations between them among highrisk and normal pregnants in Gynecology Department of Meram Medical Faculty, Selçuk University. The universe of this study constitutes of 297 pregnants who apllied to Gynecology Department of Meram Medical Faculty, Selçuk Üniversity between the dates of 15 February?31 August 2006. After giving information about subject of the search to the women and taking their approval about accepting to participate in the study, sociodemographical information form, WHOQOL-BREF (TR) and Beck Depression Inventory (BDI) were filled by face to face method. And the the answers were recorded by the researcher. The cut-off point of BDI was taken as 17 and over. BDI level was found as 17 and over among 34.4% (n=102) of all women who took part in our study and 42.6% (n=49) among high-risk pregnants. The incidence of high-risk pregnancies among the pregnants who had 17 and over BDI score was found significantly higher than the ones who took 16 and lower (p=0.017). High-risk pregnancies were 3?4 times higher among the pregnants whose husbands had education levels of primary, middle and high school than the level of university [OR, %95 CI; 3.060 (1.430-6.549), 4.474 (1.840-10.879), 3.222 (1.546-6.714)]; 1.7?2.8 times higher among the pregnants whose husbands were worker, unemployed and self-employed than the ones whose husbands were official [OR, %95 CI; 2.804 (1.382-5.689), 1.853 (0.955-3.598), 1.679 (0.434-6.495)]; 1.8 times higher among the pregnants who had 17 and over BDI score than the ones who had 16 and under BDI score [OR=1.807, %95 CI; (1.108-2.946)]; 3 times lower among employer pregnants than the unemployer ones [OR=0.316, %95 CI; (0.134-0.744)]. Quality of life scores in the domains of physical health (p<0.001), psychological health (p=0.036), social relationships (p<0.001) and general (p<0.001) were significantly lower among high-risk pregnants than normal ones. In our study, it was seen that 42.6% of high risk pregnants had depression symptoms and high-risk pregnancies diminished quality of life. Early diagnosis and treatment of depression in pregnancy matter for mother and baby. As the doctor diagnoses, he or she has to decide to give treatment or not by evaluating the rate of benefit and deficit.
