Çalışmada, total abdominal histerektomi operasyonu yapılan hastalarda abdominal fasya altına (subfasiyal) uygulanan % 0,25 levobupivakain infüzyonunun postoperatif ağrının çeşitli komponentlerine ve opiyoid tüketim miktarına etkisi araştırıldı. Çalışmaya ASA Î -Î Î risk grubundaki 50 hasta dahil edildi ve randomize olarak eşit sayıda iki gruba ayrıldılar. Operasyonun kapanma aşamasında, Grup Î ' deki hastaların rektus kaslarının üzerine iki adet kateter yerleştirildi ve elastomerik bir pompaya bağlanarak 4 ml/saat hızında % 0,25 levobupivakain infüzyonu başlatıldı. Tüm hastalara, operasyondan sonraki 1 saat içerisinde VAS' ları 4' ün altına düşene kadar 10 dakika aralıklarla 1mg kurtarma morfini yapıldı ve 1. saat sonunda morfin HKA takıldı. Grup Î Î ' deki hastalara başka bir analjezi yöntemi uygulanmadı. Hastaların ilk 1 saat içerisindeki VAS' ları ve kurtarma morfin miktarları, 1/2, 2, 6, 12, 24 ve 48. saatlerde; kesi yerinde ağrı, derin visseral ağrı, hareketle ağrı şeklinde sorgulanan VAS değerleri, DEL (HKA cihazının hastaya verdiği miktar), DEM (hastanın cihazdan istek miktarı), bulantı, kusma, verilen antiemetik miktarları, sedasyon skorları ve postoperatif 24. saatteki kortizol değerleri kaydedildi. Ayrıca oturma ve ayağa kalkma süreleri, hasta ve cerrah memnuniyetleri tespit edildi. 48. saatte kateterler çekilerek mikrobiyolojik incelemeleri yapıldı. Her iki grup arasında demografik özellikler, cerrahi ve anestezi süreleri, cerrahi tipi, cerrahi kesi tipi açısından istatistiksel anlamlı fark belirlenmedi. lk 60 dakikadaki VAS değerleri, kurtarma morfin miktarları, ½, 2, 6, 12, 24. ve 48. saatlerdeki kesi yerinde ağrı, derin visseral ağrı, hareketle ağrıları ve DEL - DEM miktarları, 1, 2, 6, 24. saatlerdeki bulantı düzeyleri ve toplam antiemetik verilme miktarları, 30, 60, 120. dakikalardaki sedasyon skorları, 24. saatteki kortizol değerleri, Grup Î ' de, Grup Î Î ' ye göre anlamlı olarak azdı. Oturma ve ayağa kalkma süreleri Grup Î ' de daha kısa sürede oldu ve hasta ve cerrah memnuniyeti bu grupta daha yüksek seyretti. Hastalarda enfeksiyona veya lokal anestezik toksisitesine dair bulguya rastlanmadı. Sonuç olarak; abdominal histerektomilerde % 0,25 levobupivakainin subfasiyal uygulamasının, postoperatif, güvenilir ve etkin analjezi sağladığı ve morfin tüketimini azalttığı saptandı.
In this study, the effect of 0.25% levobupivacaine infusion applied under abdominal fascia (subfascial) for patients undergoing total abdominal histerectomy operation on various components of postoperative pain and opioid consumption amount was examined. 50 patients in ASA Î -Î Î risk group were included in the study; and they were divided into two equal randomized groups. For patients in Group I, at closing phase of the operation, two catheters were placed on rectus muscles and 0.25 % levobupivacaine infusion at 4 ml/hr speed was started by an elastomeric pump connection. For all patients, 1 mg of rescue morphine at 10 minutes interval was applied within 1 hours after operation until VAS values dropped under 4; and at the end of first hour, morphine PCA was connected. No other analgesia method was applied for patients in Group II. Within the first hour, VAS and rescue morphine value; and at 1/2, 2nd, 6th, 12th, 24th and 48th hours, VAS value as pain in incision point-deep visceral pain-pain caused from movement, DEL (that is applied to patient by PCA machine), DEM (that is demanded by patient from the machine), nausea, vomitting, applied antiemetik amounts, sedation scores; and cortizol value at postoperative 24th hour were recorded for all patients. Moreover, seating and stand up time, patient and surgeon satisfactions were determined. At the end of 48th hour, catheters were removed and sent for microbiologic examination. No significant statistical difference among the two groups was determined for demographic properties, surgery and anesthesia durations, surgery type and surgical incision type. VAS value at first 60 minutes; rescue morphine amounts; pain at incision point, deep visceral pain, pain caused by movement and DEL-DEM amounts at 2nd, 6th, 12th, 24th and 48th hours; nausea levels at 1st, 2nd, 6th and 24th hours and total applied antiemetics; sedation scores at 30th, 60th and 120th minutes and cortizol value at 24th hour decreased significantly at patients in Group I, compared to patients in Group II. Seating and stand up times were observed to be quicker at patients in Group I; and patient and surgeon satisfaction were recorded to be higher. No enfection or local anesthesical tocsicity were encountered for the patients. As a result, it was found that subfascial application of 0.25% levobupivacaine for abdominal histerectomies provided postoperative, reliable and effective analgesia and decrease morphine consumption.
