Amaç: Deneysel sepsis modelinde antioksidan bir ajan olan NAC ve Erdostein'in serbest oksijen radikalleri ve plazma düzeylerine olan etkilerini belirlemektir. NAC ve Erdostein'in sepsisin neden olduğu organ fonksiyon bozuklukları ve akciğer doku hasarını önlemedeki rolü araştırıldı. Materyal ve metod: Çalışmada Sprague-Dawley cinsi 40 adet rat kullanıldı. Ratlar randomize 10'arlı gruplara ayrıldılar. Ratlarda çekum ligasyon perforasyon (ÇLP) yöntemiyle sepsis oluşturuldu. Sham grubu, sepsis grubu, sepsis + NAC (20 mg/kg/gün) ve sepsis + Erdostein (20 mg/kg/gün) şeklinde gruplar oluşturuldu. NAC rubunda ilaçlar, ÇLP'den sonra, 0. 8. ve 16. saatte verildi. Erdostein grubunda ise, 0.ve 12. saatte verildi. 24. saatte , lökosit, eritrosit GSH, lökosit MPO ve plazma MDA değerlerinin tayini için kan örnekleri alındı. Ayrıca histopatolojik inceleme ve doku MDA tayini için, deneklerin ölümünü takiben akciğerden doku örnekleri alındı. Bulgular: Gruplar lökosit değerleri açısından karşılaştırıldığında; grup 3'te grup 2'ye göre lökosit değerlerinde artma anlamlı bulundu (P<0.05). Grup 4'de grup 2'ye göre artma ise, anlamlı bulunmadı (P>0.05). Gruplar eritrosit GSH değerleri açısından kıyaslandığında; grup 4'te grup 2'ye göre artma istatistiksel olarak anlamlı saptandı (P<0.05). Grup 3'de grup 2'ye göre artma anlamlı saptanmadı (P>0.05). Gruplar lokosit MPO değerleri açısından incelendiğinde; grup 3 ve grup 4'de grup 2'ye göre düşme istatistiksel olarak anlamlı tespit edildi (P<0.05). Grup 3 ve grup 4'ün, grup 2'ye göre lökosit MPO değerini azaltma yönünden birbirine karşı üstünlükleri istatistiksel olarak tespit edilmedi (P>0.05). Gruplar plazma MDA değerleri açısından değerlendirildiğinde; Grup 4'te grup 2'ye göre azalma istatistiksel olarak anlamlı bulunurken (P<0.05), grup 3'de grup 2'ye göre azalma anlamlı bulunmadı (P>0.05). Gruplar akciğer doku MDA değerleri açısından karşılaştırıldığında; Grup 3 ve grup 4'de grup 2'ye göre düşme istatistiksel olarak anlamlı bulundu (P<0.05). Buna karşın grup 3 ve grup 4'ün, grup 2'ye göre akciğer doku MDA düzeyini azaltma yönünden birbirine karşı üstünlükleri istatistiksel olarak tespit edilmedi (P>0.05). Gruplar akciğer dokusundaki PMNL artışı yönünden değerlendirildiğinde; Grup 4'te grup 2'ye göre azalma görüldü (P<0.05). grup 3'de grup 2'ye göre azalma anlamlı bulunmadı (P>0.05). Gruplar akciğer dokusundaki alveolar septal kalınlaşma, konjesyon, hemoraji yönünden incelendiğinde; Grup 3ve grup 4'de, grup 2'ye göre azalma anlamlı değildi (P>0.05). 41 Sonuç: Deneysel sepsis modelinde antioksidan bir ajan olan NAC' ve Erdostein'in düşük doz uygulanmasında eritrosit GSH, lökosit MPO düzeylerine, akciğer fonksiyonlarına, plazma ve doku MDA seviyelerine olumlu etkileri mevcuttur. Düşük doz NAC ve Erdostein tedavisinin, sepsise bağlı organ fonksiyon anormalliklerini azaltmasına rağmen, bu etki histopatolojik olarak akciğer dokusuna yansımamıştır. Daha kapsamlı ve yeni çalışmalarla değişik dozlarda uygulanan NAC ve Erdostein'in sepsis tedavisinde önemi daha iyi anlaşılacaktır.
Objectives: This study was designed to determine the effects of NAC and erdosteine as antioxidant agents on the free oxygen radicals and their plasma levels. Materials and methods: In this study, 40 Sprague-Dawley rats were randomly divided into four groups as sham (n = 10), sepsis (n = 10), sepsis + NAC (20 mg/kg/24 h, n = 10) and sepsis + erdosteine (20 mg/kg/24 h, n = 10) groups. Experimental sepsis model were performed by a cecal ligation and perforation (CLP). NAC was administered at 0, 8 and 16. hours after CLP. Erdosteine was administered at 0. and 12. hours after CLP. The blood samples were taken at the 24. hours to determine the levels of leucocyte and erythrocyte GSH, leucocyte MPO and plasma MDA. And the lung tissue samples were taken also to determine tissue injury and MDA and histopathological investigation after decapitation. Findings: The leucocyte levels were significantly increased in group 3 according to the second group (P<0.05).In the 4.group, leucocyte levels were increased according to the second group, but not statistically significant (P>0.05). The erythrocyte GSH levels significantly increased in group 4 according to the second group (P<0.05).In the 3.group, erythrocyte GSH levels were increased according to the second group, but not statistically significant (P>0.05) The leucocyte MPO levels significantly decreased in group 4 according to the second group (P<0.05) but not statistically differences between together (P>0.05). The Plasma MDA levels significantly decreased in group 4 according to the second group (P<0.05). In the 3.group, plasma MDA levels decreased according to the second group but not statistically significant (P>0.05). The lung tissue MDA levels significantly decreased in groups 3 and 4 according to the second group (P<0.05) but not statistically differences between together (P>0.05). The lung tissue PMNL levels significantly decreased in group 4 according to the second group (P<0.05). In the 3.group, PMNL levels were decreased according to the second group, but not statistically significant (P>0.05). 43 In the histopathological investigation of lung tissue there was no statistically significant differences between groups 3 and 4 according to the second group about alveolar septal hypertrophy, congestion, haemorrhage) Results: In experimental sepsis model, by the administration of NAC and erdosteine as antioxidant agents at lower doses many meaningful positive effects were detected on the levels of erythrocyte GSH, leucocyte MPO, respiration function, plasma and tissue MDA. In spite of the low dose NAC and erdosteine therapies decrease the organ function abnormalities, these effects were not reflected to the histopathological investigations. These findings suggest that NAC and Erdosteine could be a possible therapeutic agent for sepsis and its mortality. However, further studies are needed to elucidate the effects of these drugs at higher doses.
