Solunumun, Kalp Hızı Değişkenliğinin (KHD) güç spektral yoğunluğundaki (GSY) HF bileşeninin oluşumunda ana etken olduğu konusunda bir konsensüs bulunmasına rağmen, son zamanlarda yapılan bazı deneysel çalışmalar LF bileşenine de etkileri olduğunu göstermiştir. Bu tez çalışmasında, solunumun KHD GSY'si üzerindeki etkisi, Ursino ve Magosso tarafından geliştirilmiş, kapsamlı bir solunum- kardiovasküler sistem etkileşimi matematiksel modeli kullanılarak araştırılmıştır. EKG ile eş zamanlı kaydedilen göğüs ve abdomen çevresi genişleyip daralma sinyalleri modele giriş olarak verilip, elde edilen KHD sinyalleri ile EKG'den elde edilen gerçek KHD sinyalleri karşılaştırılmıştır. Modelden elde edilen KHD'nin gerçekteki ile zaman ve frekans düzleminde büyük benzerlik göstermesi ve model KHD'sinin güç spektrumunda gerçek KHD'dekine benzer LF bileşenleri ortaya çıkması üzerine, bu duruma solunumun LF bölgesindeki gücünün sebep olabileceği hipotezi oluşturulmuştur. Bu hipotezi doğrulamak üzere, kardivasküler kontrol sisteminin LF bölgesi içerisinde (0.1 Hz civarında) bir rezonans bölgesi olduğu simülasyonlarla gösterilmiştir. Hipotezin deneysel olarak doğrulanması için, 9 denekten spontan soluma ve LF bölgesindeki gücü nispeten büyük olacağı öngörülen nispeten düzensiz soluma durumlarında kayıtlar alınmıştır. Deneyler, solunumun LF bölgesindeki gücünün artışına bağlı olarak, gerçek ve model KHD'lerinin LF güçlerinin arttığını göstermiştir. Böylece solunumun, KHD`nin güç spektrumundaki yalnızca HF bölgesinin oluşumunda değil, LF bölgesinin oluşumunda da önemli rol oynadığı gösterilmiştir. Bu durumda LF/HF oranının sempato-vagal dengenin göstergesi olarak kullanılamayacağına ve KHD analizlerinde solunumun durumunun da göz önüne alınması gerektiği sonucuna varılmıştır. Bu tez çalışmasının sonuçlarına dayanarak, solunum ve KHD sinyallerinin gerçek ve eş zamanlı olarak analizine imkan veren kişisel bilgisayar tabanlı bir sistem geliştirilmiştir. Bu sistem, göğüs kafesi çevresi sinyalleri veya akciğer hacmi değişimleri ile birlikte EKG sinyalini kaydedebilmekte, bir kullanıcı arayüzü vasıtasıyla, solunum ve KHD sinyallerinin, gerçek zamanlı olarak zaman ve frekans düzlemi analizlerini yapabilmektedir.
In general there is consensus on the idea that respiration is the main contributor to the generation of the HF (high frequency) peak of Heart Rate Variability (HRV) Power Spectral Density (PSD). On the other hand some studies indicate that respiratory parameters can also affect the LF (low frequency) peak of HRV PSD. In this study, Ursino and Magosso?s cardiovascular system (CVS) model, which also includes mechanisms of respiration-CVS interactions, is used to investigate effects of respiration on the power spectrum of HRV. This respiration-CVS model is a very comprehensive differential equation based, non-linear model. In this study chest and abdomen circumference signals are recorded simultaneously with ECG and are incorporated into the Ursino and Magosso model. The predictions of the model regarding HRV are compared with the actual HRV calculated from the ECG. Based on both the time domain and the frequency domain similarities, in the LF region, between Real and Model HRV, it is hypothesised that the more low frequency (LF) power there is in the respiratory signal, the more LF power there is in both the model predicted HRV and the actual HRV. To verify this hypothesis, it is shown by simulations that the baroreflex feedback system has its maximum gain at its natural frequency at about 0.1 Hz (which is in the LF region). Experiments on nine volunteers are also performed for spontaneous breathing, and also for slightly irregular breathing for which the respiratory signals have relatively more LF power. It is observed that the more LF power there is in the respiratory signals, the more LF power there is in both the model and the real - HRVs. It is concluded in general that respiration not only is the major contributor to the genesis of the HF peak in HRV power spectrum, but it also plays an important role in the genesis of its LF peak. Thus the LF/HF ratio, which is used to assess sympathovagal balance, cannot be correctly utilized in the absence of simultaneous monitoring of respiration during an HRV test. Based on conclusions of this study, a personal computer based, real time respiration and HRV monitoring and analysis system is developed. This system records chest circumference or lung volume signals together with ECG and has a user interface for real time display of the time and frequency domain parameters of both respiration and HRV.
