Bu çalışmanın amacı, sigara içen kronik periodontitisli hastalarda cerrahisiz periodontal tedavi ve bu tedaviye ek olarak diyod lazer ile subgingival dekontaminasyon uygulamasının, klinik periodontal parametreler ve dişeti oluğu sıvısında (DOS) bulunan interlökin-1β (IL-1β), total antioksidan seviyesi (TAS), total oksidatif seviye (TOS) ve protein karbonil seviyesi üzerine etkisinin incelenmesidir. Çalışmaya 30 ile 60 yaşları arasında, günde en az 15 adet sigara içen, sistemik olarak sağlıklı, 15 gönüllü kronik periodontitis hastası kabul edildi. Hastaların tüm ağız sondlama cep derinliği, klinik ataşman seviyesi, plak, gingival ve sondlamada kanama indeksleri kaydedildi. Daha sonra split mouth dizaynda DOS örnekleri alındı, diştaşı temizliği yapıldı ve oral hijyen eğitimi verildi. Bir sonraki seansta, tüm ağız mekanik tedavi işlemi sonrası test uygulanacak bölge randomize olarak belirlendi ve test bölgesine diyod lazer ile periodontal cep dekontaminasyonu uygulandı. Tedavi sonrasında 1. ve 3. aylarda DOS örneklemeleri ve klinik periodontal kayıtlar tekrarlandı. DOS'taki IL-1β, TAS, TOS ve protein karbonil düzeyleri ELISA yöntemi ile belirlendi. İstatistiksel analizler sonucunda çalışmada uygulanan tedaviler ile her iki grupta da, ataşman seviyesi dışındaki tüm klinik periodontal parametrelerde başlangıca göre iyileşme ve DOS hacminde azalma sağlarken (p<0,05), ataşman seviyesinde istatistiksel olarak anlamlı fark gözlenmedi (p>0,05). DOS'ta TAS çalışma süresince artış göstermekle beraber bu artış grup içi değerlendirmelerde istatistiksel olarak anlamsızdı (p>0,05). Test ve kontrol grupları karşılaştırıldığında DOS IL-1β,TAS, TOS ve protein karbonil düzeyleri arasında fark gözlenmedi (p>0,05). Bu çalışmanın sınırları içinde, sigara içen hastalarda cerrahisiz periodontal tedaviye ek olarak diyod lazer ile dekontaminasyon uygulamasının klinik periodontal parametreler, IL-1β, TAS, TOS ve protein karbonil düzeyleri üzerine ek fayda sağlamadığı sonucuna varıldı.
The aim of this study was to investigate the effects of non-surgical periodontal therapy and adjunctive use of diode laser decontamination in chronic periodontitis patients via the clinical periodontal parameters and gingival crevicular fluid (GCF) levels of interleukin-1β (IL-1β), total antioxidant status (TAS), total oxidant status (TOS), and protein carbonyl. The study group consisted of 15 smoker volunteers who were diagnosed with chronic periodontitis. Volunteers smoke minimum 15 cigarettes a day and they were between ages of 30 and 60 and systemically healthy. Prior to the periodontal treatment, whole mouth probing depth, clinical attachment level, plaque index, gingival index and bleeding on probing index scores were recorded for each patient. After GCF sampled in split mouth design, scaling was performed and oral hygiene instructions were given to all participants. In the next session, the whole mouth root planning was performed and then the test region was determined by randomization. Adjunctive periodontal pocket decontamination is applied to the test region with a diode laser. Following the treatment, clinical periodontal measurements and GCF samplings were repeated on the 1st and 3rd months. The IL-1β, TAS, TOS and protein carbonyl levels in the GCF were determined by ELISA method. Statistical analysis of the data revealed that the treatment modalities applied in both groups improved the clinical parameters and reduced the GCF volume (p<0,05), except the attachment level (p>0,05). Despite observing a continuous increase in the GCF TAS levels, intragroup differences were not significant (p>0,05). GCF IL-1β, TAS, TOS and protein carbonyl level changes in test group were not significant when compared to control group (p>0,05). Within the limits of this study, we concluded that non-surgical periodontal treatment and the adjunctive use of diode laser decontamination in smoker chronic periodontitis patients has no any additional benefits for clinical periodontal parameters and GCF IL-1β, TAS, TOS and protein carbonyl levels.
