İskemi reperfüzyon hasarı temel olarak iskemik dokuda ve perfüzyon yeniden sağlandıktan sonra doku oksijen ile tekrar karşılaştığında ortaya çıkan serbest oksijen radikallerinin (SOR) oluşturduğu hasarların tümüdür. Bu deneysel çalışmada, bir sıçan modelinde oluşturulan iskelet kası iskemi reperfüzyon hasarında magnezyum infüzyonunun etkinliği, biyokimyasal ve histopatolojik parametreler yardımı ile araştırıldı. Deneysel çalışma Sprague Dawley cinsi 20 yetişkin erkek sıçan üzerinde yapıldı (Kontrol grubu n=10, deney grubu n=T0). Tüm deneklerde 50 mg/kg ketamin HCI i.m ile genel anestezi uygulandı. Tüm deneklerde alt karın bölgesi insizyon ile açıldıktan sonra periton da açıldı ve retroperitoneal alanda her iki iliak ve femoral venlere ayrılmadan önce abdominal aorta eksplore edilerek dönüldü. Bu bölgede vena kava inferior da eksplore edildi. Sıvı replasmanı için operasyon başında v. kava inferior'a 12G i.v. katater konularak infüzyon pompası yardımıyla; kontrol grubundakilere deney süresince % 0,9'luk izotonik 0,01 ml/g/saat dozunda, deney grubundakilere ise % 0,9'luk izotonik içine magnezyum sülfat (% 15, 10 ml/ampul) konularak total 0,5 mg/100g dozunda magnezyum iskemi ve reperfüzyon dönemleri boyunca gidecek şekilde infüzyon yapıldı.
Serum Mg levels were not changed at the end of both periods in control group, however, in study group it was increased in both periods when compared with the preoperative Mg levels. After the occurrence of injury IL-8 levels were increased in the control group, whereas no difference was observed in the study group. However, there were no differences in MDA levels in both groups, which is also considered as a marker of ischemia-reperfusion injury. According to histopathological examinations it has been concluded that edema, neutrophil infiltration, eosinophilia and disappearance of sitriation can be regarded as significant and earlier parameters in evaluation of ischemia-reperfusion injury. Neither edema nor neutrophil infiltration can be avoided after Mg infusion in study group. Moreover, eosinophilia and disappearance of sitriation were significantly increased in the study group when compared with the control group. According to these results, histopathologically, Mg infusion was regarded unsuccessful in the prevention of ischemia-reperfusion injury. In conclusion, it has been shown that magnesium infusion is partially effective for preventing the ischemia-reperfusion injury when compared according to biochemical parameters. However, histopathologically, the tissue injury that is triggerred in ischemia-reperfusion periods can not be avoided by magnesium infusion.
