İntihar ya da tamamlanmış intihar kişinin kendi yaşamına kendi isteğiyle son vermesidir. İntihar davranışı, intihar ve intihar girişimleri için kullanılan genel bir tanımdır. Dünyada her yıl bir milyon intihar ve on milyon intihar girişimi meydana gelmektedir. Her ne kadar intihar önlenebilir bir halk sağlığı problemi olmasına rağmen, intihar riskini belirlemeye yönelik az sayıda objektif yöntem vardır. Serotonin (5-hydroxytryptamine, 5-HT) santral sinir sisteminde bir nörotransmitterdir ve S100B protein beyinde fazla miktarda bulunan asidik bir kalsiyum bağlayıcı proteindir. Son yıllarda pek çok fizyolojik görevi ve farklı nöropatolojik durumlarda önemi olması nedeniyle S100B proteinine ilgi artmaktadır. Daha önce yapılan çalışmalarda postmortem S100B proteini ve serotonin düzeylerinin intihar düşüncesi ve intihar açısından belirteç olarak kullanılmasının yararlı olabileceği belirtilmiştir. Bu çalışmada, intihar (n = 32) ve intihar dışı (n = 56) adli tıbbi otopsi olgularından (n = 88, ölümden en fazla 24 saat geçmiş olanlar) alınan beyin omurilik sıvılarında S100B proteini ve serotonin düzeyleri araştırılmıştır. S100B proteini ve serotonin analizi için beyin omurilik sıvısı örnekleri toplanmış ve analiz edilinceye kadar -80ºC'de saklanmıştır. S100B proteini ve serotonin konsantrasyonları tüm örneklerde ELISA (Enzyme Linked Immunosorbent Assay) yöntemi ile tespit edilmiştir. Serebrospinal sıvıda S100B protein seviyeleri intihar olgularında, intihar dışı olgulara göre yüksek (sırasıyla 9.3 ± 2.9 ng/ml ve 5.4 ± 2.0 ng/ml), serotonin seviyeleri ise intihar olgularında, intihar dışı olgulara göre düşük bulunmuştur (sırasıyla 10.4 ± 4.9 ng/ml ve 19.0 ± 5.7 ng/ml) (P<0.05). S100B proteini ve serotonin düzeyleri ile cinsiyet, yaş grupları, postmortem interval ve ölüm nedeni arasında korelasyon bulunamamıştır. Çalışmamız sonucunda intihar olgularında tespit edilen S100B proteini yüksekliği ve serotonin düşüklüğünün intihar riski belirlenmesinde birlikte kullanılabilecek uygun biyokimyasal belirteçler olduğu düşünülmüştür. Elde edilen verilerin farklı toplumlarda ve yaş gruplarında geniş kapsamlı çalışmalarla desteklenmesi ve geliştirilmesinin gerektiği sonucuna varılmıştır.
Suicide or suicide completion is the act of taking one?s own life voluntarily, and usually intentionally. Suicidal behavior is a general term used to refer to suicide and suicide attempts. About one million suicides and ten million suicide attempts occur worldwide each year. Although suicide is a preventable public health problem, there are few objective assays for suicide risk. Serotonin (5-hydroxytryptamine, 5-HT) is a neurotransmitter in the central nervous system and S100B protein is an acidic calcium-binding protein which is most abundantly found in the brain. In recent years, there has been a proliferation of interest in the protein S100B, its many physiological roles and its behavior in various neuropathological conditions. Previous studies have suggested the usefulness of the postmortem S100B protein and serotonin levels as markers of suicide ideation and suicide. In this study, we investigated S100B protein and serotonin levels in the cerebrospinal fluid (CSF) in medicolegal autopsy cases (n = 88, within 24 hours postmortem), including those of suicide cases (n = 32) and non-suicide cases (n = 56). For S100B protein and serotonin analysis, cerebrospinal fluid samples were collected and were stored at -80ºC. The S100B protein and serotonin concentrations were measured in all samples by ELISA (Enzyme Linked Immunosorbent Assay) method. The CSF S100B levels were higher in suicide group than non-suicide group (9.3 ± 2.9 ng/ml versus 5.4 ± 2.0 ng/ml), and serotonin levels were lower in suicide group than non-suicide group (10.4 ± 4.9 ng/ml versus 19.0 ± 5.7 ng/ml) (P<0.05). There was no correlation between S100B protein and serotonin levels with gender, age groups, postmortem interval and cause of death. It is concluded that both S100B protein and serotonin in cerebrospinal fluid may be used as biochemical markers for determination of suicide risk. We realize that further prospective studies are needed to assess a broad clinical applicability of these biomarkers for suicidality in different populations and age groups.
