Bu çalışmanın amacı, önemli bir mortalite ve morbidite nedeni olan KOAH'ın akut alevlenmesinde kullanılan antibiyotiklerin, akılcı ilaç kullanımı ilkeleri doğrultusunda reçete edilişlerini değerlendirmektir. Bu çalışmada Konya Selçuk Üniversitesi Tıp Fakültesi Göğüs Hastalıkları Polikliniğine başvurmuş olup, araştırmaya katılan KOAH akut alevlenmesi tanısı konmuş ve reçetelerinde Makrolid, Kinolon, B-Laktam ve bunların kombinasyonları ile tedavi yoluna gidilmiş 16-90 yaşları arasındaki 275 hasta değerlendirilmiştir. Çalışmaya katılan hastalar, tanımlayıcı bilgilerinin ve mevcut durum hakkındaki bilgilerini içeren soruların bulunduğu bir anket formu doldurmuş ve tedavi başladıktan sonra 3., 5., 7. ve 14. günlerde hastalar telefon viziteleri ile izlenenerek anketteki sorular sorulmuştur. Toplanan sonuçlar analiz edilerek çalışmada kullanılan antibiyotik gruplarının (Makrolidler, Kinolonlar, B-Laktam ve bunların kombinasyonları) etkinliği, uygunluğu, güvenirliği ve maliyeti değerlendirilmiştir. Çalışma sonunda en az tercih edilen ikili kombine ilaç gruplarının tedavi sonu iyileşmeyi en fazla sağlayan ve aynı zamanda en maliyetli tedaviyi sağlayan ilaç grubu olduğu görülmüştür. En çok tercih edilen Makrolid grubu ilaçlar ise tedavi sonunda, tüm sepmtomlardaki azalma oranı ve tüm semptomlardaki iyileşme oranı en yüksek tekli ilaç grubudur. Tüm ilaç grupları arasında kamu maliyeti en düşük, ortalama maliyeti ise en düşük ikinci ilaçtır. Etkinlik, güvenirlik, uygunluk ve maliyeti birlikte değerlendirerek yaptığımız çalışmada akılcı ilaç ve dolayısıyla akılcı antibiyotik kullanımı açısından en verimli ilaç grubunun diğer tekli ilaç gruplarına göre daha etkin ve daha az maliyetli olan Makrolid grubu olduğu görülmüştür. Sonuç olarak; akut KOAH alevlenmesinde, hekimlerin doğru endikasyon, doğru ilaç ve uygun maliyet bakımından rasyonel ilaç ve rasyonel antibiyotik kullanımı ilkelerine uygun davrandıkları gözlenmiş ve daha disiplinli ilaç politikaları ile hem toplum içi sağlık eğitimi hem de hekim ve eczacıların meslek öncesi ve meslek içi eğitimlerine ağırlık verilerek akılcı (rasyonel) ilaç – antibiyotik kullanımı bilincinin artırılmasının önemi anlaşılmıştır.
The purpose of this study is to evaluate the prescribing of the antibiotic, used for COPD acute exacerbation which is an important mortality and morbidity reason, in the direction of rational drug use principles. In this study, 275 patients between 16-90 years old were evaluated. Those patients applied to Konya Selçuk University, Medical Faculty, Pulmonology Polyclinic and participated in the research. The patients were diagnosed COPD acute exacerbation and were treated with Macrolide, Quinolon, B-Lactam and their combinations. The participating patients filled out a questionnaire with the questions consisting of descriptive information and present condition information. The patients were called by phone and were asked the questions in 3rd, 5th, 7th and 14th days after the treatment had started. The results were analyzed and the efficiency, the sufficiency, the reliability and the cost of the used antibiotic groups (Macrolides, Quinolos, B-Lactam and their combinations) were reviewed. The study results show that the least preferred binary-combined drug groups provided the highest recovery following the treatment and the most costly treatment drug group. The most preferred Macrolid group of drugs are the single-drug group and the recovery rate and the decrease rate are the highest in all symptoms. It is the lowest public cost drug of all drug groups and the second drug with the lowest average cost. In our study, it is seen that, in terms of rational drug use/rational antibiotic use, the most efficient drug group with a less cost is the Macrolid group comparing to the other single-drug groups evaluating through the efficiency, the reliability, the sufficiency and the cost. As a result, regarding the acute COPD exacerbation, it is observed that the physicians behaved accordingly to the principles of rational drug/antibiotic use in respect to accurate indication, medication and relevant cost. The importance of raising the awareness on rational drug-antibiotics use is understood concentrating on public health education and prevocational and vocational training of physicians and pharmacists with a most disciplined drug policy.
