Amaç: Periostin havayolu epitelinden ve akciğer fibroblastlarından, IL-4 ve IL-13 aracılığı ile salgılanan ekstraselüler matriks proteinidir. T helper 2 aracılı allerjik inflamasyonda rolü olduğu ve allerjik astımlı hastalarda arttığı gösterilmiştir. Bu çalışmada, astımlı çocuk hastalarda serum periostin ve soluk havasında nitrik oksit (FeNO) düzeylerinin değerlendirilmesi amaçlandı. Gereç ve yöntem: Çalışmaya 7-17 yaş arası 60 astımlı (29 allerjik, 31 allerjik olmayan) ve 30 sağlıklı kontrol toplam 90 çocuk (45 kız, 45 erkek) dahil edildi. Hastaların demografik verileri, eosinofil yüzdesi, deri prick testi, solunum fonksiyon testi ve spesifik IgE düzeyleri kaydedildi. FeNO ve serum periostin düzeyi ölçüldü. Bulgular: Çalışmaya alınan tüm bireylerde serum periostin değerleri 6-12 yaş grubunda (n:58) (ort±SD) 59.08±12.79 ng/dl, 13-18 yaş grubunda (n:32) 46.18±11.64 ng/dl saptandı. Serum periostin değerlerinin yaşla anlamlı korelasyonu olduğu ve yaşla birlikte azaldığı görüldü (p<0.001). Serum periostin düzeyi allerjik astımlı hastalarda 52.10±13.93 ng/dl olup, allerjik olmayanlar (54.61±13.05 ng/dl) ile istatistiksel olarak anlamlı fark saptanmadı. FeNO değerleri hasta ve sağlıklı kontrol grubu ile karşılaştırıldığında farklı değildi (15.5 ppb ve 14.0 ppb, p=0.486). Allerjik astımlı hastalarda total eosinofil sayısının ortalaması % 3.52±2.15, allerjik olmayanlarda ise % 2.28±1.88 olup istatistiksel olarak anlamlıydı (p< 0.05). Hasta grubunda sadece allerjik hastaların NO ve periostin değerleri arasındaki korelasyon değerlendirildiğinde istatistiksel olarak anlamlılık olmasa da p değeri anlamlılığa yakın saptandı (r: 0.327 p= 0.077). Ayrıca, serum periostin düzeyi polisensitize olan astımlı çocuklarda, sadece polen duyarlılığı olan grupla karşılaştırıldığında p değeri anlamlılığa yakın saptandı (58.2±15.1 ve 44.65±8.3, p=0.065) Sonuç: Serum periostin değerinin akciğer dışından özellikle kemik ve mezenkimal dokulardan kaynaklanması, büyüme ve gelişme döneminde olan astımlı çocuklarda biyobelirteç olarak kullanımını sınırlandırmaktadır. Bununla birlikte polisensitize olan grupta serum periostin düzeylerinin yüksek olması çoklu allerjen duyarlılığının havayolunda eosinofilik inflamasyonu arttırdığını düşündürmektedir.
Aim: Periostin is an extracellular matrix protein secreted by airway epithelium and pulmonary fibroblasts, and the secretion is mediated by interleukin (IL)-4 and IL-13. Periostin was shown to play a role in T helper 2-mediated allergic reactions and to be increased in the patients with allergic asthma. This study evaluated the serum periostin levels and exhaled nitric oxide (FeNO) in children with asthma. Materials and Methods: A total of 60 children with asthma (29 of them were allergic, and 31 of them were nonallergic) and a control group comprising 30 healthy children, ages between 7 and 17 years, were included in the study. Demographic data, eosinophil percentages, skin prick test, respiratory function test and specific IgE levels of the patients were recorded. The levels of FeNO and serum periostin were measured. Results: The serum periostin level was (mean±SD) 59.08±12.79 ng/dL in the 6–12 age group (n = 58) and 46.18 ± 11.64 ng/dL in the 13–18 age group (n = 32). The serum periostin level significantly correlated with age and decreased with increased age (p<0.001). The serum periostin level was 52.10±13.93 ng/dL in patients with allergic asthma,and no statistically significant difference was detected from the nonallergic (54.61±13.05 ng/dL). The FeNO values were not different when the patient and the control groups were compared with each other (15.5ppb and 14.0 ppb, p=0.486). The mean of total eosinophil count (%) was 3.52±2.15 in patients with allergic asthma and 2.28±1.88 in nonallergic individuals, and it was statistically significant (p<0.05). When the correlation between NO and periostin values of the only allergic patients in the patient group was considered, p value was detected as closer to significance level even if no significance existed statistically (r = 0.327,p=0.077). Also, the p value was detected as closer to significance level when the serum periostin levels in polysensitized children with asthma and children with only pollen sensitivity were compared with each other (58.2±15.1 and 44.65±8.3, p=0.065). Conclusion: Secretion of serum periostin by bone and mesenchymal tissues, besides the lungs, limits its use as a biomarker in children with asthma who are in growth and development stages. However, increased levels of serum periostin in polysensitized children give rise to the thought that multiple allergen sensitivity increases eosinophilic inflammation in the airways.
