Çalışmamızın amacı üst çenenin yatay yön yetersizliklerinin karışık dişlenme döneminde akrilik genişletme apareyi ile yapılan yelpaze tipi ve geleneksel tip hızlı üst çene genişletmesinin diş-çene ve yüz yapıları üzerine etkilerinin incelenmesidir. Çalışmamız iskeletsel Sınıf I veya II olan hastalarda, üst çenede sadece ön bölgede darlık gösteren vakalar ile çift taraflı posterior çapraz kapanış gösteren vakalar üzerinde yapılmıştır. Yelpaze tipi genişletme grubundaki 20 hastanın yaşlarının ortalaması 8.96±1.19 yıl, geleneksel tip hızlı genişletme grubundaki 22 hastanın yaşlarının ortalaması 8.69±0.66 yıldır. Yelpaze tipi genişletme grubu için tedavi süresi ortalama 19.77±2.02 gün ve geleneksel tip hızlı genişletme grubu için ortalama 22.82±2.52 gündür. Her iki genişletme prosedürü için vida ilk hafta günde 2 ×1/4 tur ve akabinde günde 1/4 tur çevirtildi. Daha sonra 3 aylık pekiştirme tedavisi uygulanmıştır. Hastaların lateral ve frontal sefalometrik radyografilerinin yanında alt ve üst çenenin alçı modellerini de inceledik. İstatistiksel değerlendirmede tekrarlayan ölçümlerde varyans analizi ile grup içi değişimlerin tespiti için `Eşleştirilmiş örneklerde t testi' ve gruplar arası değişimlerin karşılaştırılması için `Bağımsız örneklerde t testi' kullanıldı. Her iki genişletme işleminden sonra üst çene öne ve aşağıya doğru hareket gözlenmiştir. Burun boşluğu ve üst çene kaidesi genişliklerinin artışında geleneksel tip genişletme daha fazla ve kalıcı olmuştur. Yelpaze tip genişletme grubunda üst keser dişlerde belirgin bir ileri itim meydana gelmiştir. Her iki genişletmede köpek dişleri arasındaki mesafe artışı benzerken, geleneksel tip genişletme grubunda azı dişleri arasındaki genişlik artışı daha fazladır.
The aim of this study was to investigate the effects of fan-type rapid and rapid maxillary expansion with modified acrylic bonded expansion appliance on dentofacial structures in the treatment of transverse maxillary deficiency in the mixed dentition period. Some patients were Class I skeletal relationship and others were Class II skeletal relationships. The fan-type rapid expansion group had anterior constricted maxillary width with normal intermolar width and the rapid expansion group had bilateral constricted maxillary width. The fan-type rapid expansion group is consist of 20 patients at the mean age of 8.96±1.19 years and the rapid expansion group is consist of 22 patients at the mean age of 8.69±0.66 years. The treatment time is meanly 19.77±2.02 days for fan-type rapid expansion group and 22.82±2.52 days for the rapid expansion group. The patients were instructed to activate the screw 2×¼ per day for first week and followed by 1×¼ per day for both groups. Dental casts of both jaws and lateral and frontal cephalometric radiographs were taken before and after expansion and three months after completing for each patient. The data firstly compared using `Repeated Measurements Analysis of Variance? than `Paired samples t test? was used to evaluate the treatment and retention effects and `Independent samples t test? was used to consider the differences between two groups. Maxilla had moved downward and forward in both groups after expansion therapy. The nasal cavity and maxillary width were more expanded in the rapid expansion group than the fan-type rapid expansion group and there was only a few relaps in the rapid expansion group. There was significant labial tipping of the upper incisors in the fan-type rapid expansion group. The expansion of intercanine width was similar in both groups, but the expansion of intermolar width was significantly greater in the rapid expansion group than the fan-type rapid expansion group.
