Epilepsi sık görülen kronik nörolojik hastalıklardan biridir. Epilepside sık olarak görülen komorbiditeler depresyon ve yorgunluktur. Epileptiklerde yorgunluk ve depresyon hayat kalitesini etkileyerek ciddi morbiditelere sebep olabilir. Epilepside depresyon ve yorgunluk sık görülmekte, katkıda bulunan faktörler belirsizliğini korumaktadır. Çalışmamızda epilepside eser element anormallikleri olup olmadığı; yorgunluk ve depresyonla eser elementlerin ilişkisi; ve eser elementlerin yorgunluk ve depresyon gelişimine katkısı olup olmadığı araştırıldı. Çalışmamıza 48 epileptik olgu ve 39 kontrol olmak üzere toplam 87 kişi alındı. Çalışmaya kabul edilen kişilerin sabah 8.00-9.00 saatleri arasında aç karına, herhangi bir antikoagulan içermeyen vakumlu tüplere venöz kan örnekleri alındı. Santrifüj edilip serumları ayrılarak eser element ölçümleri için -80° C derin dondurucu saklandı. Çalışma günü tek seansta ve aynı ekip tarafından serumdan Pb, Zn, Cu, Mn ve Se eser element düzeyleri ölçüldü. Tüm katılımcılara aynı kişi tarafından Beck Depresyon Ölçeği, Yorgunluk Şiddet Skalası ve SF-36 Yaşam Kalitesi skalaları uygulandı. İstatistiki değerlendirme için SPSS for Windows 18,0 programı kullanıldı. Depresyon ve yorgunluk yüzdeleri hesaplandı. Sonuçlar ortalama ± standart sapma olarak belirlenip, gruplar karşılaştırıldı ve p<0,05 ise anlamlı kabul edildi. Yine eser element düzeyleri, depresyon ve yorgunluk arasında korelasyon olup olmadığı araştırıldı.Epileptiklerin yaş ortalaması 29,21± 8,67, kontrollerin 26,41 ± 6,99 olarak hesaplandı. Gruplar karşılaştırıldığında istatistiksel olarak yaş bakımından anlamlı bir fark saptanmadı (p> 0.05). FSS ve BDÖ skorları epileptiklerde anlamlı yüksek bulundu (p<0,05). Epileptiklerde depresyon oranı %35,4; yorgunluk oranı %45.8 belirlendi. Se, Cu, Mn düzeyleri epileptiklerde anlamlı yüksekti (p<0,05), fakat Zn ve Pb düzeylerinde anlamlı fark bulunmadı (p>0,05) . Alt grup analizlerde depresyonu ve yorgunluğu olan ve olmayan epileptiklerin karşılaştırılmasında da eser element düzeylerinde anlamlı fark bulunmadı (p>0.05). Eser element düzeyleri ile Beck Depresyon Ölçeği ve Yorgunluk Şiddet Skalası arasında anlamlı korelasyon saptanmadı (p>0,05). Çalışmada yorgunluk ve depresyon arasında orta düzeyde pozitif korelasyon saptanmıştır (r=0,346, p=0,016). Epileptiklerde yorgunluk ve depresyonun daha sık olarak görüldüğü; epileptiklerde eser element plazma seviyelerinde anormallikler olabileceği; eser elementlerin yorgunluk ve depresyonu olanlar ve olmayanlar arasında anlamlı fark göstermediği ve eser elementlerin yorgunluk ve depresyonla belirgin bir korelasyon göstermediği belirlenmiştir. Yorgunluk ve depresyon arasında orta düzeyde pozitif korelasyon saptanmış olup; yorgunluk gelişmesinde eser element düzeylerindeki değişikliklerin katkısının olabileceği, ancak en önemli değişkenin depresyon olduğu ileri sürülebilir.
Epilepsy is one of the frequent chronic neurologic diseases. Frequent comorbidities in epilepsy are depression and fatigue. Depression and fatigue on epilepsy patients may cause severe morbidities affecting of their quality of life. Depression and fatigue are frequent in epilepsy and coefficients which cause them are still uncertain. In our study, existance of trace element abnormalities on epilepsy patients; relationship in between fatigue and depression and trace elements; and comorbidity of trace elements on development of depression and fatigue are investigated. Our research included 87 people which consist of 48 epileptic cases and 39 control cases. Venous blood samples of individuals with morning fasting that participate to our study were taken to vacuumed tubes without any anticoagulant agent between 08.00-09.00 am. Serum of these samples were separated with centrifuge and kept in a freezer with -80 C for trace element evaluations. Trace element levels of Pb, Zn, Cu, Mn and Se were evaluated in serum with the same team in one session on study day. Beck Depression Scale (BDS), Fatigue Severity Scale (FSS), Profil of Mood State Scale (POMS) and SF-36 Quality of Life Scale were performed on all participants by the same person. SPSS for Windows 18,0 was used for statistical assessment. Depression and fatigue percentages were evaluated. Results were determined as ± standart deviation, groups were compared and p<0.05 was accepted as significant. Correlation between trace elements, depression and fatigue was investigated. Mean age of epileptic patients were 29.21±8.67 y and mean age of control patients were 26.41±6.99 y. In accordance with ages, there was no statistically significant difference between groups (p>0.05). FSS and BDÖ scores were significantly high on epileptic patients. Depression rate of epileptic patients was 35.4 % and fatigue rate was 45.8 % . Se, Cu, Mn levels were significantly high on epileptic patients (p<0.05); however there was no significant difference on Zn and Pb levels (p>0.05). There was no significant difference on trace element levels in the comparison of epileptic patients with and without depression and fatigue in subgroup analyses (p>0.05). There was no significant correlation between trace element levels and Beck Depression Scale and Fatigue Severity Scale (p>0.05). Moderate level positive correlation was determined between depression and fatigue in this study (r=0.346, p=0.016). It was determined that fatigue and depression are more frequent in epileptic patients; there might be abnormalities of plasma levels of trace elements in epileptic patients; there was no significant difference of trace elements between patients with and without fatigue and depression and trace elements had no significant correlation with depression and fatigue. Moderate level correlation was determined between fatigue and depression; changes in trace element levels might affect the development of fatigue, but it can be suggested that the most important variable is depression.
