Bu çalıĢmanın amacı; 1- bir hemostatik ajan (Ankaferd Blood Stopper-ABS) ve bir self etch adeziv sistemin (Clearfil Protect Bond-CPB) direk kuafajda kullanımından sonra oluĢan pulpa cevabının belirlenmesi, 2- Pulpa perforasyonun sebep olduğu nemliliğin ve ABS kontaminasyonunun CPB‟un bağlanma dayanımının devamlılığı üzerine etkilerinin incelenmesidir. Ġlk amaç için; çekim endikasyonu konmuĢ, 21 çürüksüz insan üçüncü molar diĢi seçildi. Perforasyon içeren Black I kaviteler açıldı. Grup 1‟de kanama steril pamuk pelet ile durduruldu ve kalsiyum hidroksit (CH)+ZnOE siman+amalgam ile restore edildi. Grup 2‟de kanama steril pamuk pelet ile durdururlduktan sonra CPB ve kompozit rezin ile restore edildi. Grup 3‟te kanama ABS ile durdurulduktan sonra CPB ve kompozit rezin ile restore edildi. DiĢler 90 gün sonra çekildi, formalinde fikse edildi ve histolojik inceleme için hazırlandı. Pulpa cevabı enflamatuar cevap, yumuĢak doku organizasyonu, tamir dentin formasyonu ve bakteriyel boyanma açısından değerlendirilmiĢtir. Veriler Kruskal-Wallis testi kullanılarak istatistiksel analize tabi tutuldu. Ġkinci amaç için; mikrotensile bağlanma dayanımı testi için derin Black I kaviteler açıldı. Örnekler pulpa perforasyonu içeren ve içermeyen, rastgele iki ana gruba ayrıldı. Her grupta örneklerin yarısı ABS ile kontamine edildi ve tüm örnekler CPB ve kompozit rezin ile restore edildi. Tüm gruplardaki örneklerin yarısı 24 saat ve 12 aylık periyotlar için sınıflandırıldı. On iki aylık gruptaki örnekler nemli ortamda ve simüle pulpal basınç altında saklandı. Saklama süresi sonund tüm örnekler mikrotensile bağlanma dayanımı testine tabi tutuldu. Veriler Kruskal-Wallis ve Mann Whitney U testlerine tabi tutuldu. Pulpal cevapta tüm değerlendirme kriterleri açısından fark bulunamadı (p>0,05). Bağlanma dayanımı çalıĢmasında ise kontamine ve kontamine olmayan gruplar arasında anlamlı fark bulundu (p<0,05). ABS, CPB‟un dentine olan bağlanma dayanımını düĢürmüĢtür. Ancak her iki saklama süresinde de perforasyonlu ve perforasyonsuz gruplar arasında fark bulunamamıĢtır. ABS direk pulpa kuafajında kanama durdurucu olarak kullanılabilir. CPB direk pulpa kuafajında kullanılabilir. ABS kullanıldığında dentin mümkün olduğunca az kontamine edilmelidir.
The aims of this study; 1- It was to investigate the human dental pulp response after the use of a hemostatic agent (Ankaferd Blood Stopper-ABS) and a self-etching adhesive system (Clearfil Protect Bond-CPB) in direct pulp capping, 2- It was to investigate the effect of pulp perforations and ABS contamination on the bond durabiliyt to dentin of CPB. For the first aim; 21 non-carious human third molar teeth scheduled for extraction were selected. Class I cavities with pulp exposures were prepare. In group 1: bleeding was controlled with sterile cotton pellets and restored with calcium hydroxide (CH) + ZnOE cement + amalgam. In group 2: bleeding was controlled with sterile cotton pellets and restored with CPB and a composite resin. In group 3: bleeding was controlled with ABS and restored with CPB and a composite resin. The teeth were extracted after 90 days, formalin-fixed and prepared for histological investigation. The pulp response was categorized using the following criteria: inflammatory response, soft tissue organization, reparative dentine formation and bacterial staining. Data were submitted to statistical analysis, using nonparametric Kruskal-Wallis test. For the second aim; deep Class I cavities in molar teeth were prepared for microtensile bond strength test.The specimens were randomly assigned to two main groups, with and withouth pulp perforations. Half of the specimens in each group were contaminated with ABS and all specimens were restorated with CPB and a composite resin. Then the teeth in all groups were assigned to two for 24 hours and 12 mounth periods. Spescimens of 12 mounth group were stored in a moist conditon under simulated pulpal pressure. After storage, all specimens submitted to microtensile bond strength test. The data was analyzed Kruskal-Wallis and Mann Whitney U tests. In pulp response study it wasshowed no statistically significant diferences between groups for all parameters (p>0,05). In the dentin bond strength study it was showed statistically significant difference among contaminated and non-contaminated groups (p<0,05). ABS reduced microtensile bond strength to dentin of CPB. But it was showed no statistically significant differences among groups with and withouth perforation and among two time periods. ABS can be used as hemostatic agent in direct pulp capping treatment. CPB can be used as direct pulp capping material. When using ABS, dentin to be contaminated as little as possible.
