Amaç: Biz bu çalışmada Tip II Diabetes Mellitus'lu hastalarda yaşam kalitesini, depresyon semptomatolojisini ve ilişkili faktörleri araştırmayı amaçladık. Gereç-yöntem:10.11.2014 ile 31.03.2015 tarihleri arasında Selçuk Üniversitesi Aile Hekimliği Diyabet Eğitim Polikliniğine başvuran erişkin tip 2 diyabetes mellitus hastalığı olan, takip ve tedavi altında olan 440 hasta alındı. Katılımcıların sosyodemografik özelliklerini içeren anket formu, SF-36 yaşam kalitesi anketi, BDÖ yüz yüze görüşme yöntemi ile uygulandı. DM'ye bağlı daha önce tanı konmuş göz, nörolojik ve nefrolojik komplikasyonlarının olup olmadığı hasta dosyalarından son bir yıllık verileri retrospektif taranarak değerlendirildi. Tüm veriler SPSS 16.0 istatistik paket programına kaydedildi. Verilerin değerlendirilmesinde Student-t testi, Ki-kare, ANOVA varyans analizi ve Pearson korelasyon analizi kullanıldı. Bulgular: Kadın hastalarda erkek hastalara göre SF-36'nın tüm alt boyutlarında puan ortalamaları istatiksel açıdan anlamlı derecede düşük bulundu (p<0,0001).BDÖ puan ortalaması kadın hastalarda erkek hastalara göre istatiksel açıdan anlamlı derecede yüksek bulundu (p<0,0001).Yaşlı hastalarda genç erişkin hastalara göre (p<0,024) ve yaşlı hastalarda erişkin hastalara göre (p=0,004) fiziksel fonksiyon puan ortalamaları istatiksel açıdan anlamlı derecede düşük bulundu.VKİ'si normal olan hastalarda obez hastalara göre fiziksel fonksiyon (p<0,0001), fiziksel rol güçlüğü (p<0,0001), ağrı (p=0,002), enerji (p=0,05) puan ortalamaları istatiksel açıdan anlamlı derecede yüksek bulundu. İlkokulu bitirememişlerde ilköğretim mezunlarına, lise mezunlarına, üniversite ve üzeri mezunlarına göre SF-36'nın tüm alt boyutlarında puan ortalamaları istatiksel açıdan anlamlı derecede düşük bulundu (p<0,0001). BDÖ puan ortalamaları üniversite ve üzeri mezunlarında ilkokulu bitirememişlere göre (p<0,0001) ve ilköğretim mezunlarına göre (p=0,009) istatiksel açıdan anlamlı derecede düşük bulundu.Evli olanlarda dul veya boşanmışlara göre fiziksel fonksiyon (p<0,0001), ağrı (p=0,05), genel sağlık (p<0,0001), enerji (p=0,023), mental sağlık (p=0,027) puan ortalamaları arasında istatiksel açıdan anlamlı derecede yüksek bulundu. BDÖ puan ortalaması dul veya boşanmış hastalarda evli hastalara göre istatiksel açıdan anlamlı derecede yüksek bulundu (p<0,0001). Ev hanımlarında fiziksel fonksiyon, fiziksel rol güçlüğü, genel sağlık, enerji puan ortalamaları diğer meslek gruplarına göre istatiksel açıdan anlamlı derecede düşük bulundu (p<0,0001). Çalışmamıza alınan hastaların BDÖ puan ortalaması ev hanımı hastalarda diğer meslek grubu hastalara göre istatiksel açıdan anlamlı derecede yüksek bulundu (p<0,0001). Geliri düşük olanlarda fiziksel fonksiyon puan ortalamaları geliri orta (p=0,003) ve geliri iyi olanlara (p=0,002) göre istatiksel açıdan anlamlı derecede düşük bulundu. BDÖ puan ortalaması geliri düşük olanlarda geliri orta (p=0,014) ve iyi olanlara (p=0,040) göre istatiksel açıdan anlamlı derecede yüksek bulundu. 1–10 yıl diyabet süresi olanların fiziksel fonksiyon (p<0,0001), fiziksel rol güçlüğü (p=0,001), genel sağlık (p<0,0001), sosyal fonksiyon (p=0,006), emosyonel rol güçlüğü (p=0,007), mental sağlık (p=0,025) puan ortalamaları 20 yıl ve üzeri diabet süresi olanlara göre istatiksel açıdan anlamlı derecede yüksek bulundu. Diyabet süresi 20 yıl ve üzeri olanların diyabet süresi 1–10 yıl olanlara göre BDÖ puan ortalamaları istatiksel açıdan anlamlı derecede yüksek bulundu (p=0,008). OAD kullananlarda insülin+OAD kullanlara göre fiziksel fonksiyon, fiziksel rol güçlüğü, ağrı, genel sağlık, sosyal fonksiyon puan ortalamaları istatistiksel açıdan anlamlı derecede yüksek bulundu (p<0,0001). Hiç komplikasyonu olmayanlarda 2 ve üzeri komplikasyonu olanlara göre fiziksel fonksiyon puan ortalamaları (p<0,0001) istatistiksel açıdan anlamlı derecede yüksek bulundu. BDÖ puan ortalamaları (p=0,044) hiç komplikasyonu olmayanlarda 2 ve üzeri komplikasyonu olanlara göre istatistiksel açıdan anlamlı derecede düşük bulundu. Sonuç: Yaşam kalitesi ve depresyon semptomatolojisi; kadınlarda, yaşlılarda, obezlarda, eğitim düzeyi düşük olanlarda, dul veya boşanmış olanlarda, ev hanımlarında, geliri düşük olanlarda, diyabet süresi uzun olanlarda, insülin kullananlarda, 2 ve üzeri komplikasyonu olanlarda daha kötüdür. Diyabetli bireylerde yaşam kalitesi ve depresyon semptomatolojisini etkileyen birçok tıbbi ve sosyodemografik faktörler olup, hem sağlık çalışanları hem de hastalar bu konuya gerekli önemi vermelidirler.
Objective: We aimed to investigate the quality of life and depression symptomatology situations and the related factors in patients with Type II Diabetes Mellitus in this study. Material - Methods: Between 10/11/2014 and 31/03/2015, 440 adult patients with type 2 diabetes mellitus, and under treatment admitted to Selcuk University Family Medicine Outpatient Diabetes Education clinic were included in the study. A questionnaire containing sociodemographic characteristics of the participants, the SF-36 quality of life questionnaire was applied with BDI face to face interviews. Whether they had previously diagnosed eye, neurological or nephrological complications connected with DM were evaluated retrospectively scanning the last year data from the patient files. All data were recorded into the SPSS 16.0 statistical software package. Student-t test, chi-square, ANOVA analysis of variance and Pearson correlation analysis were used to evaluate the data. Results: Mean scores of females in all SF-36 subscales were statistically significantly lower than those of male patients (p<0.0001). BDI mean scores were statistically significantly higher in female patients compared to male patients (p<0.0001). Physical function scores in elderly patients were statistically significantly lower than in young adult patients (p<0.024) and in adult patients (p=0.004). Physical function (p<0.0001), physical role limitations (p <0.0001), pain (p=0.002) and energy (p=0.05) average scores were statistically significantly higher in patients with normal BMI than those of obese patients. The mean scores in all SF-36 subscale were statistically significantly lower in people not graduated from elementary school than in primary school graduates, high school graduates, university graduates and above (p<0.0001). Mean BDI scores in university graduates and above was statistically significantly lower than than those not graduated from primary school (p<0.0001) and graduated from primary school (p=0.009). Physical function (p<0.0001), pain (p =0.05), general health (p<0.0001), energy (p=0.023) and mental health (p=0.027) mean scores in married patients were statistically significantly higher than in widowed or divorced patients. The average BDI scores were statistically significantly higher in widowed or divorced patients than married patients (p <0.0001). Physical function, physical role limitations, general health and energy mean scores were statistically significantly lower in housewives compared to other occupation groups (p <0.0001). Mean BDI scores of the housewife patients included in our study were statistically significantly higher than patients in other professional groups (p<0.0001). The mean physical function score (p=0.003) in patients with lower income was significantly lower than those with middle and higher income (p=0.002). The mean BDI scores in the patients with low income was significantly higher than of the people with middle income (p=0.014) and those with high income (p=0.040). Physical function (p<0.0001), physical role limitations (p=0.001), general health (p<0.0001), social function (p=0.006), emotional role limitations (p=0.007), mental health (p<0.0001) mean scores of the patients with 1-10 years duration of diabetes were found statistically significantly higher than those with 20 years and over duration of diabetes. The mean BDI scores of the patients with 20-year and over duration of diabetes were statistically significantly higher than those with 1-10 years duration of diabetes (p=0.008). Physical function, physical role limitations, pain, general health and social function mean scores in patients using OAD was statistically significantly higher compared to patients using insulin + OAD (p<0.0001). The average physical function scores (p<0.0001) of the patients with no complications were statistically significantly higher than those with 2 and more complications. The mean BDI scores (p=0.044) of the patients with no complications were statistically significantly lower than those with 2 and more complications. Conclusion: Quality of life and depression symptomatology; are worse in females, the elderly, the overweight, people with lower level of education, in the widowed or divorced, housewives, those with low incomes, those with longer duration of diabetes, patients using insulin and those with 2 or more complications. There are many medical and sociodemographic factors affecting quality of life and depressive symptomatology in the individuals with diabetes, so both health care workers and patients should pay the necessary attention to this issue.
